11| to be included on certain form; exempting candidates |
| for federal office from certain requirements; |
12| requiring Secretary of State Election Board to |
| prescribe certain form; and declaring an emergency. |

16|AMENDMENT NO. 1. Page 1, Lines 6 through 11 1/2, strike the title |
| to read as follows: |
